Membership agreement

 

MONTHLY Membership Terms: I, the undersigned, understand that:

  • Membership Term:  I may cancel my membership at any time by providing a written notice with an original signature on the form provided by EBB Dance no less than 10 business days prior to the first day of the calendar month of membership cancellation.
  • Membership Dues:   Dues will be withdrawn from my credit card account on file at the first of every month.   If I choose to pay with an alternate form of payment then I will do so on or before the first of the month.   Late payment of monthly membership fees will incur a $35 late fee for every month past due.
  • Rollover Classes:  I understand that any unused classes from Tier I and Tier II Memberships may be used for up to one (1) year from the date of issue.  In the event of cancellation or termination, I understand that I will forfeit all unused rollover classes, and will not receive compensation in any form.
  • Cancellation:  Only active memberships may be cancelled, and cancellation does not relieve me of my obligation to pay any fees or penalties incurred before the cancellation takes effect.   To cancel I need to submit my written request with original signature on the form provided by EBB Dance no less than 10 business days prior to the calendar month of cancellation.   I understand that any outstanding balance on my account must be paid in full before the cancellation will take effect.
  • Freeze:   In the event that I need to put my membership status on hold, I need to submit my request no less than 10 business days prior to the first day of the calendar month of membership freeze, and I agree to pay a freeze fee of $10/month. I understand that my account cannot be cancelled while it is frozen. I further understand that while my membership is frozen, all membership discounts and privileges with respect to group classes, workshops, reward programs, and the like will not be accessible, and that I will need to purchase classes and lessons individually if I wish to participate.
  • Downgrade:   If I choose to downgrade my membership, I need to submit my written request with original signature on the form provided by EBB Dance no less than 10 business days prior to the first day of the calendar month of membership downgrade. I further agree that when downgrading, I will forfeit 50% of any accumulated rollover classes, calculated per month, rounded down to the nearest whole number; remaining rollover classes will retain their original expiration date.
  • Upgrade:   If I choose to upgrade my membership, I can upgrade my membership at any time by submitting a written request with original signature on the form provided by EBB Dance. When I upgrade, I will retain any accumulated rollover classes with their original expiration date.
  • Transfer:  Member classes, perks, and the like cannot be transferred to another person.
  • Security and Confidentiality:   I understand that in order to maintain privacy and security, I am the sole person who may make changes to my membership, contact information, and credit card on file. I further understand that it is my responsibility to keep accurate, updated contact and credit card information on file.

 

All Memberships are priced per individual; no family or couple options are available at this time.

NOTE:  Out of respect for the privacy of our students and the security of their information, each individual is responsible for any changes to his/her account, including but not limited to the contact information, card on file for payment, changes to the level of membership, freezes, and cancellations.  We are not able to discuss a student’s account with any individual except the registered student, except in the case of students who are under 18 years old.
